Patients have the right and responsibility to control decisions regarding the use or refusal of medical care, including life-prolonging treatment, nutrition and hydration.

Advance directive forms are available at Altru Hospital in the Social Work department, from Pastoral Services, from the Medical Records Department, from the Hospital Supervisor, or from Altru Home Services staff.
