Prevention Clinic
Utilizing scientific-based health risk assessments and access to a specialized physician, health and wellness specialist and dietitians, all participants are evaluated and leave with a personal action plan directing them on the next steps to appropriate lifestyle changes to become healthier.
All dimensions of wellness are discussed: nutrition, physical activity, stress level, tobacco and alcohol use, vehicle safety, biometric measures and questions specific to women’s and men’s health.
Appointments begin with a Family Health Portrait and TRALE health risk assessment by a Health and Wellness Specialist. Patients then visit with a specialized physician for a full interpretation of the health history and risk assessment. The physician will also provide individualized and customized recommendations based on the results. Lastly, patients receive nutrition and exercise recommendations from a dietitian and health and wellness specialist, respectively.
All services are offered for a one time, nominal fee.
