Patient-Centered Care

​Patient-centered means that you are the focus of your healthcare team.

A patient-centered care team begins with your primary care provider (personal physician) and encompasses a team of professionals working together to provide comprehensive medical care.

Who does a Patient-Centered Care Team include?

  • Patient and Family
  • Primary Care Provider
    • Responsible for leading the team
    • Collaborates with the patient and team in developing goals and care plan
  • Office Nurse/Medical Assistant
    • Active member of the team
    • Notifies primary care provider and health coach of patient questions
  • Health Coach
    • Develops ongoing relationship with the patient
    • Acts as liaison between the patient and their healthcare team
    • Active in patient education, goal setting, and self-management support for patients with chronic diseases

Other members of your Healthcare Team may include a social worker, pharmacists, panel specialist, patient access specialists, medical specialists, behavioral health specialists and other roles as identified.

Coordination of your care may include:

  • Primary care
  • Hospital care
  • Specialty care
  • Home health care
  • Community services and resources

Role of Patient-Centered Care Team

  • Provide comprehensive and evidence based care to manage your healthcare needs
  • Listen to your healthcare concerns
  • Develop goals through shared decision making
  • Explain your care plan
  • Answer your healthcare questions
  • Coordinate your healthcare needs, including prescriptions, lab work and specialty care
  • Respect your cultural background and language preferences
  • Encourage your involvement in your healthcare plan
  • Provide education and self management support

  • Follow the plan that you and your patient-centered care team developed
