Employee COVID-19 Screening Tool
Any Altru employee who has traveled internationally or domestically in the past 14 days must notify his or her leader and submit the Employee COVID-19 Screening Questionnaire to email@example.com. Employees must be cleared by employee health prior to returning to work.
Employee Health will contact the employee via the phone number provided on the questionnaire to review the information. They will inform the employee and the leader whether the employee is:
- able to return to work immediately
- needs to stay home and quarantine
- or needs to report to the community hotline for symptoms
Please note that if an employee exhibits any symptoms of cold or flu, fever, cough or shortness of breath while quarantined at home, the employee needs to contact the community hotline immediately (701.780.6358) and contact Employee Health as this could change the date of return to work.
The CDC advises people to stay home for a period of 14 days from the time the person left an area with widespread or ongoing community spread of COVID-19. This is a must for all Level 3 areas and some of the Level 2 areas. Click here for assistance in travel making decisions.
