Altru Now Provides Services at LaGrave on First

Grand Forks, N.D. – Altru Health System is now providing services and resources to care for the residents of LaGrave on First, permanent housing for the homeless in Grand Forks.

The expanded programming at LaGrave on First offers multi-disciplinary services to include a family medicine physician one day a week, a full-time onsite RN Case Manager, therapy services, a medical financial services representative, and telemedicine options for some medical specialty services.

LaGrave on First opened in fall 2018 with 42 one-bedroom private living spaces. This permanent supportive housing for homeless individuals has the potential to help reduce overall homelessness in our community. To complement the Housing First approach endorsed by the federal government, Altru is dedicated to providing supportive services and stabilizing the opportunities to receive care. “Altru recognizes the health determinants of the residents at LaGrave on First. The focus of our services will be on providing a supportive environment, convenient access to care, assistance with health and social services navigation, medication management explanations, benefits counseling, and more,” states Brad Wehe, CEO of Altru.

The services offered on-site at this location are available to LaGrave on First residents and at this time is not available to non-residents. “Providing services to these residents is a unique effort. As a community-owned health system, we are striving to develop ways to make healthcare easily accessible to patients in our community who may not receive care without this type of offering,” says Jodi Sorum, Director of Population Health at Altru Health System.
