Altru Health System Joins Grand Forks Public Health to Conduct Community Health Assessment

Posted OnOctober 07, 2019

Altru Health System and Grand Forks Public Health are excited to announce a community-driven process to determine the health concerns of residents throughout the region. The 2019 Community Health Survey, a comprehensive collection of data regarding health issues, available services, and opportunities for action in our communities is open now and available until October 16, 2019. This survey, part of the Community Health Assessment, will identify health issues of concern and provide critical information to those in a position to make an impact on the health of our region including governments, social and human service agencies, businesses, healthcare providers, and other entities as well as consumers.

This community health survey, with the purpose of gathering information that will be used to make our community healthier, is being made possible by a unique collaboration between Altru Health System, Grand Forks Public Health, and the University Of North Dakota Masters Of Public Health Program. The survey is open to anyone over 18 years old in Grand Forks and Polk Counties. The survey is confidential and can be accessed on the Altru Health System website and Grand Forks Public Health’s website or a paper copy can be obtained at Grand Forks Public Health, 151 S. 4th St. Ste. N301 or the information desk at Altru Hospital, 1200 S. Columbia Rd. With questions about the survey, please contact Ashley Evenson, MPH at 701.777.6368.
