Medication Therapy Management

All our pharmacists are qualified to provide Medication Therapy Management (MTM). Through this service, you can receive comprehensive education about all your medications, information about potential drug interactions, and recommendations about the most affordable medication options for you. Your pharmacist can also provide wellness services such as immunizations and health education.

MTM is especially helpful for patients who have multiple chronic conditions, patients who are taking multiple types of medications, or patients who are taking medications that require close monitoring.

For patients with Medicare and with most insurances, MTM is free.

Comprehensive Medication Review (CMR)

One service included in MTM is a comprehensive medication review, or CMR. This involves a meeting with your pharmacist, either in person or on the phone, to review all the medications you’re currently taking, including over-the-counter drugs and vitamins or supplements. Many supplements and non-prescription drugs can interact with prescription medications, causing adverse side effects. For example, some supplements can enhance the effect of your prescription drug, potentially causing an overdose.

During a CMR, your pharmacist will talk with you about the effects of all your medications and supplements, as well as the ways they interact with each other. They will talk with you about any side effects you experience and any difficulties you have with taking all your medications on time. If you have trouble keeping up with all your medications, your pharmacist will work with you to find solutions.

Target Medication Review (TMR)

Sometimes, you may have trouble refilling your medication as often as you’re supposed to, or you may be prescribed two medications that do the same thing. Situations like this could trigger a target medication review, or TMR. If there’s something unusual in your medication use or history, your insurance will flag it and your pharmacist may contact you. During your target medicine review, your pharmacist will discuss any concerns or challenges you have with taking your medications and help correct them.
