Patient-Centered Care

Patient-centered means that you are the focus of your healthcare team. The patient-centered care team begins with your primary care provider (personal physician) and encompasses a team of professionals working together to provide comprehensive medical care.

Patient-Centered Care Team Includes:

  • Patient and Family
  • Primary Care Provider

    • Responsible for leading the team

    • Collaborates with the patient and team in developing goals and care plan

Office Nurse/Medical Assistant

  • Active member of the team

  • Notifies primary care provider and health coach of patient questions

Health Coach

  • Develops ongoing relationship with the patient

  • Acts as liaison between the patient and their healthcare team

  • Active in patient education, goal setting, and self-management

  • support for patients with chronic diseases

Other Members of your Healthcare Team:
