One way to make those decisions is by completing a healthcare directive. Healthcare directives, including those from residents of other states, are honored at Altru Hospital and can often be written while a patient is in the hospital.
Advance directive forms are available at Altru Hospital in the Social Work department, from Spiritual Care, from the Medical Records Department, from the Hospital Supervisor, or from Home Health staff.
